Who can join Care Access Health Plan?

Care Access provides services to both individuals and employer groups. Our coverage, benefits and low cost are ideal for people who are uninsured, students, part-time workers, dependents and all individuals regardless of immigration status.

What Counties does Care Access Health Plan Service?
Care Access serves all of Miami-Dade, Broward and Palm Beach counties. The Company plans to expand statewide.

What Services are covered by Care Access Health Plan?
Care Access covers  wide range of comprehensive and routine medical services with an emphasis on preventive care. Hospital services are excluded under the Plan, however, supplemental hospital and dental plans are offered through select carriers. Care Access benefits include Primary, Pediatric and Specialty Physician care, as well as Urgent Care, Prescription Drugs, Diagnostic Services, Ambulatory Surgical Services, Behavioral Health, Hearing and Vision Services. Do you offer Hospital and Dental Coverage?
While hospital services are excluded under our health plan, supplemental hospital and dental plans are available through select carriers.

What is the Prescription Drug Benefit?
Each member can receive up to $100 per month ($1,200) per year in prescription drugs. Generic Drug co-pays are as low as $7. A $100 per month benefit is also included for allergy treatment and there is a $100 per month benefit for Injectibles. How many times can I visit my Primary Care Physician or take my child to the Pediatrician?
Visits to Primary Care Providers within the Care Access network (see provider directory) are unlimited. Co-payments apply. Please see the Benefits section of our Website to view the Schedule of Benefits for specific coverage detail.

Can my child join, even if I do not?
Yes. We accept children from birth, with or without their parents. As applicants are medically underwritten each can be viewed on an individual basis for acceptance.

Do you need a minimum number of employees or members to join this plan?
No. There is no minimum number of employees or members required to join Care Access.

Can you accommodate group billing for a company?
Yes. With Care Access, an employer may pay all, a portion, or none of the premium for his/her employees. List billing is also available. The Plan is also portable, meaning individuals may take the Plan with them if they leave their place of work.

How do you review groups in terms of acceptance?
All groups will be accepted or declined, as a group. Care Access reserves the right to impose pre-existing limitations for certain conditions and certain individuals for a period of up to 24 months.

Is there a pre-existing clause?
Yes. If the underwriting department considers a condition as pre-extisting, certain treatments and/or medication pertaining to that condition may not be covered for a period of up to 24 months, at the discretion of the Plan.

Do I need a Social Security Number to Join?
No social security card is necessary. We accept members regardless of immigration status. Coverage is based on application (Statement of Health) and underwriting assessment.

Can I nominate my physician to be a part of the Plan?
Yes. Simply fill out a  Provider Nomination Form and submit to Care Access when you sign up. We can not guarantee that your physician will choose to join Care Access, and of course, s/he will have to pass our credentialing requirements, but we will notify you if s/he signs on as a provider with us.

How long does it take to enroll?
Once you have submitted your application, the enrollment process will begin and can take from 45 to 60 days. All applications are subject to approval. Get a free quote and enroll on-line now!

Are Chiropractic and Mental Health services covered?
Chiropractic services are unlimited and require no referral. Our Mental Health benefit includes up to 20 visits per year.

Which services require a referral from Care Access?
Most specialty physicians services and such services as Diagnostic, Home Health, Ambulatory Surgical Services, Rehabilitative Services and Durable Medical Equipment require referrals from your PCP and authorization from our UM Department.

Do I have to live in South Florida to qualify for Care Access?
Yes. You must reside within our three county service area to enroll in Care Access.

For eligibility or benefits, verification, to order ID cards or to change your Primary Care Provider, call: 1-866-429-2882.

When are premiums due?
Premiums are due thirty (30) days prior to the first day of the month of the coverage period. For example, for April 1st coverage, your premium is due March 1st; Because we are a pre-paid health plan, you may receive an invoice for the second month of your coverage before your effective date with the Plan. This is how a pre-paid health plan operates.
